Do I give it the keyword, or does it pick one?

Either. Name the keyword and it optimises for that. Leave it out and it will work out what the page is currently closest to ranking for — usually from Search Console — and tell you what it chose before it starts.

Will it rewrite my whole page?

Only if you ask. The default is surgical: title, meta description, headings, internal links and the gaps in coverage. Ask for a full rewrite and it will draft one.

Can it publish the changes to my site?

No. On-page changes come back as text for you to apply. Publishing is limited to articles, to a CMS you have connected, and only once a person approves the draft.
